Privacy Statement
In this Privacy Statement we explain how we handle your personal data. We make clear what personal data we collect and for what purposes the data is being used. We also explain how you can view, modify or delete your data. Our PLN Foundation deems it to be important that you are well informed about this and recommends that you read this statement carefully.
1. Responsibility
The PLN Foundation – located at Leeuwetand 9, 1775 BS Middenmeer – is a public benefit institution (ANBI / RSIN no. 851879950) by and for PLN carriers. Through our websites we offer all kinds of “services” and activities for fellow sufferers and relatives. In doing so, we process privacy-sensitive information, also known as “personal data”. In some cases, we work together with national hospitals, other (inter)national institutes or companies to conduct research.
2. Use of personal data
When using our “services”, activities and websites, you provide us with certain information, or personal data. We do not always process all the information below. Depending on the “services” and functionalities you use on our websites, we may process the following personal data:
- First and last name
- Company data
- Email address
- IBAN / Bank account number
- Information that you enter yourself in an open field, such as a forum post or a message in the contact form.
- Mobile and/or telephone number
- Location data
- Information about your health (condition, treatments, events, stages of illness and relationship to patient…)
3. Purposes
We process the above mentioned data for one or more of the following purposes (also depending on which “services” or functionalities you use):
- General contact in the context of a question, advice or personal story
- General contact in the context of voluntary activities
- General contact in the context of your application for a research, meeting or other events
- General contact in the context of a single or periodic donation
- Handling of donations and donation agreements
- Information via e-mailings / newsletters
- Telephone contact about “services”, activities and functionalities
Our PLN Foundation also processes personal data if we are legally obliged to do so, such as data required for our tax return.
4. Distribution to third parties
We pass on the information you provide to third parties, IF necessary for the performance of certain activities (e.g. investigations) or the handling of specific “services”. For example, we also use a third party to process online payments (Mollie) via our donation form. If we provide your data to a third party, we will ensure, among other things (with an editor agreement), that your data will not be used for other purposes. We also agree that your data will be deleted as soon as they are no longer needed.
Furthermore, we will not disclose the information you provide to any other party, unless required or permitted by law. For example, the police may request information from us as part of fraud investigations. In that case we are legally obliged to provide this information.
5. Profiling and Tracking Cookies
Our PLN Foundation combines your personal data from various sources to offer you the best online experience. We also combine this information with the information you have left behind on, or via, one of the PLN Foundation’s websites:
The PLN Foundation uses functional, analytical and tracking cookies. A cookie is a small text file that is stored in the browser of your computer, tablet or smartphone when you first visit the above mentioned websites. These are used with purely technical functionality to optimize the websites and to track your surfing behavior so that we can offer customized content. You can also delete all information previously stored in the settings of your browser at any time.
6. View, modify and delete data
You have the right to access, correct or delete your personal data. In addition, you have the right to withdraw your consent to the processing of data or to object to the processing of your personal data by the PLN Foundation and to data transferability. This means that you can submit a request to us to send the personal data that we hold about you in a computer file to you or another organisation named by you. You can send a request to inspect, correct, delete, transfer data about you or to withdraw your consent or objection to the processing of your personal data to info@plnheart.org.
To ensure that you have requested access to your personal data, we ask you to enclose a copy of your identity document alongside the request. In this copy, blacken your passport photo, MRZ (Machine Readable Zone, the strip with numbers at the bottom of the passport), passport number and Burgerservicenummer (BSN) to protect your Privacy. We will respond to your request as soon as possible, within 4 weeks. The PLN Foundation would also like to point out that you have the opportunity to submit a complaint to the national supervisory authority, Autoriteit Persoonsgegevens.
7. Third party websites
This Privacy Statement does not apply to third party websites that are “linked” to our websites. We cannot guarantee that these websites will handle your personal data in a reliable or safe manner. Therefore, before using the websites concerned, we recommend that you always read the Privacy Statement of that website for more information about the way in which it handles your data.
8. Revisions
The PLN Foundation reserves the right to this Privacy Statement from time to time. Changes will be published on our websites. We will never use your information for purposes other than those for which you have given it without prior consent. It is advisable to consult this Privacy Statement regularly so that you are aware of any changes. This version is dated 23 May 2018.
The PLN Foundation takes the protection of your data seriously and takes appropriate measures to prevent misuse, loss, unauthorised access, unwanted disclosure and unauthorised alteration.
